Using your CHC Benefit Plan


When Visiting a Physician / Provider:

Locate a participating PPO provider convenient to you. (Review your Member ID card and contact your provider network for any questions regarding participating providers and facilities).

Schedule an appointment and present your ID card when you arrive.

Your provider will call CHC to verify coverage and determine your out-of-pocket expense.

Please note that some providers may require you to pay any remaining balance after the applicable benefit and discount have been applied at the time of service.
